Build a dynamic form builder
Kliv is an AI app builder. Describe the dynamic form builder you need — conditional fields, versioned forms, phone submissions, photo attachments, routing, and reports — and it builds it.
Just enter your idea into the text box and AI will build it for you
What is Kliv?
Kliv isn’t a form SaaS product with a fixed submission table — it’s an AI app builder that builds software for you.
You describe the app you need in plain words, and Kliv builds it: the form builder, database, submission rows, routing rules, attachments, dashboards, integrations, and access rules. You own the result, run it on your own accounts, and keep changing it later by asking for changes. It isn’t a template with form fields added.
On this page, that app is a dynamic form builder for operations teams. Kliv builds all kinds of web apps; this is one for forms whose answers need to trigger work.
Why build your own form builder?
Many form tools stop at the submission. Answers arrive in email or a spreadsheet, and your team still has to route the issue, prove who followed up, and connect the answer to a building, job, or client.
Kliv builds the forms and consequences together. Forms are records your team designs and versions. Submissions are rows in the same database as your buildings, techs, jobs, and follow-ups.
What the system knows
A form builder is several kinds of record behind one interface.
The form library
Vehicle checklists, boiler inspections, incident reports, and client sign-offs can each be records with owners, statuses, and history. Retiring a form does not orphan its submissions.
Fields and conditions
If someone answers “leak,” severity and photo fields can appear. If they answer “none,” those fields stay hidden. The condition is part of the form definition.
Versions
Publishing v3 does not rewrite v2 submissions. Every submission remembers which form version it answered.
Submissions as rows
Submissions can join to building, technician, and job records. Reports can query them instead of parsing attachments.
Routing rules
An out-of-range pressure reading can open a follow-up task and post to a maintenance channel, while routine passes file quietly.
Attachments
Photos from the field can land in the file store against the submission, with the same access rules as the rest of the app.
An example: Priya’s maintenance forms
Here’s how one business might use it. It’s only an example — you would describe your own forms, field rules, alerts, submissions, and reports.
The operations lead builds the form
Priya creates a boiler-inspection form with readings, checkboxes, and a photo slot, without filing a development ticket.
A condition catches risky answers
If pressure falls outside the safe band, a required note and photo appear. The rule lives in the form definition, not in a training binder.
The new version publishes cleanly
The form publishes as v3. Twelve technicians get it next time they open the app, while the 214 older submissions keep their v2 shape.
A field answer routes itself
A technician submits an out-of-range reading from a basement. The answer opens a follow-up task and posts to Slack before he reaches the stairs.
Quarter-end reports use the rows
Priya asks for failed checks by building this quarter and sees three buildings account for most of them.
Describe the form builder you want to build
Kliv builds from your description, so the more detail you give, the closer the first version. Include who creates forms, what conditions do, where submissions go, and what alerts or reports matter.
Inspection forms
Conditional fields, photos, alerts, and versions.
“Build a dynamic form builder for our maintenance company. Let operations staff create boiler inspection forms with conditional fields, required photos, and versioning; let technicians submit on phones; open a follow-up task and post to Slack when pressure is outside 12 to 18 psi; and report failed checks by building each quarter.”
Client intake forms
Branching intake and accountant checklists.
“Build an intake form system for our accounting firm. Branch questions by business type, save each intake as a client record, create a checklist for the assigned accountant, and preserve every intake version used for tax-year review.”
Field service reports
Site checklists, signatures, and weekly summaries.
“Build a job-report form for our electricians. Include per-site checklists, required before-and-after photos, customer signature capture, job and technician links, and a weekly summary emailed to each property manager.”
How it comes together
Rows can have walls. Technicians can see their own submissions, managers can see all submissions, and a client can see only forms about their buildings. The rules apply on every read.
Consequences can happen immediately or on a schedule. A Slack post can fire when a risky answer is submitted, and a Friday digest can list unresolved follow-ups by building. Integration credentials can be kept in a vault rather than pasted into code.
Managers can ask in plain words. An agent can answer from the rows that manager may already read, and the exchange can be saved as a transcript.
The schema can grow later. If you need per-building compliance deadlines or another concept, describe it and the app can add the database changes as part of the conversation.
A form builder is often the first internal tool an operations team asks for. What teams build next covers the larger pattern.
Common questions
What exactly is Kliv?
Kliv is an AI app builder. You describe the form system you need, it builds the app, and you own the result.
Do I get a real app, or a template?
A real app. Kliv builds the form builder, database, routing rules, access rules, dashboards, and integrations around your workflow.
Can I change it after it’s built?
Yes. You can add field types, conditions, reports, integrations, access roles, or new records later.
Can non-technical staff build forms?
Yes. The builder can be a page in your own app, with field types, conditions, and publishing for the person who owns the paperwork.
What happens to old submissions when a form changes?
Each submission stores the form version it answered. Older data keeps its original shape while the new version collects the new fields.
Can people outside the company fill forms?
Yes. Clients can get sign-ins and see only the forms and submissions that concern their own buildings or work.
Can a submission trigger things outside the app?
Yes. A submission can post to Slack, send email on your templates, or open items in tools such as Trello or Linear.
Can photos be required only for certain answers?
Yes. Conditional logic can make a photo, note, or severity scale appear only when the earlier answer requires it.
Can managers query submissions later?
Yes. Submissions are rows, so reports can answer questions like failed boiler checks by building and quarter.
Can I use my own domain?
Yes. The app can run on your own domain.
Can I move off Kliv later?
Yes. The code syncs to your own Git repository, so leaving is a git pull rather than a rebuild.
